Pay Range: $32-$34/hour BOE & Depending on License

General Purpose

The Admissions Nurse (LVN/RN) is responsible for coordinating the admission process, conducting clinical assessments, communicating with referral sources, and ensuring residents transition smoothly into the facility while maintaining compliance with all regulatory requirements.
Essential Duties & Responsibilities
  • Coordinate and facilitate resident admissions from hospitals and community referrals
  • Conduct admission assessments and review clinical documentation for accuracy and appropriateness
  • Communicate with hospitals, physicians, case managers, residents, and families regarding admissions and care needs
  • Verify orders, medications, and required documentation prior to admission
  • Collaborate with interdisciplinary team members to ensure continuity of care
  • Ensure admissions are completed in compliance with federal, state, and facility guidelines
  • Assist with room setup and resident orientation upon arrival
  • Accurately document admissions information in PointClickCare and other systems
  • Participate in care planning and discharge coordination as needed
  • Maintain positive relationships with referral partners and healthcare providers
  • Support marketing and census development efforts through exceptional customer service
Qualifications

Education & Experience
  • Previous skilled nursing or long-term care experience preferred
  • Admissions or case management experience preferred
  • Experience with PointClickCare (PCC) preferred

Licensure & Certifications
  • Current California LVN or RN license in good standing
  • Current CPR/BLS certification preferred
