We're all about quality care, teamwork, and making sure our long-term residents get the best support possible. If you love paperwork (yes, someone has to!), assessments, and being a clinical detective, this is your chance to shine.

💡 What You'll Be Doing:
  • Coordinate and complete accurate Minimum Data Set (MDS) assessments
  • Focus on long-term residents—get to know the people behind the paperwork
  • Collaborate with interdisciplinary team members to ensure compliance and excellent resident care
  • Use your keen eye for detail to capture clinical information and support quality outcomes
  • Maintain documentation that aligns with federal and state regulations

What We're Looking For:
  • At least 1 year of experience as an LVN (required)
  • Prior MDS experience a big plus (but we're open to training the right person!)
  • Strong assessment, documentation, and organizational skills
  • A compassionate, team-oriented mindset
  • Preferred schedule: Wednesday-Friday, but we're flexible for the right fit
