Oak River Rehab in Anderson, CA is looking for a
Part-Time MDS Nurse to join our dedicated team!
We're all about quality care, teamwork, and making sure our long-term residents get the best support possible. If you love paperwork
(yes, someone has to!), assessments, and being a clinical detective, this is your chance to shine.
💡 What You'll Be Doing: - Coordinate and complete accurate Minimum Data Set (MDS) assessments
- Focus on long-term residents—get to know the people behind the paperwork
- Collaborate with interdisciplinary team members to ensure compliance and excellent resident care
- Use your keen eye for detail to capture clinical information and support quality outcomes
- Maintain documentation that aligns with federal and state regulations
✅ What We're Looking For: - At least 1 year of experience as an LVN (required)
- Prior MDS experience a big plus (but we're open to training the right person!)
- Strong assessment, documentation, and organizational skills
- A compassionate, team-oriented mindset
- Preferred schedule: Wednesday-Friday, but we're flexible for the right fit
