Position Overview:

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic team. As an LVN at Oak River Rehabilitation Center, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional rehabilitation services.

Key Responsibilities:
  • Provide direct patient care under the supervision of a Registered Nurse (RN) or physician.
  • Administer medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
  • Monitor and document patient progress, including vital signs, symptoms, and responses to treatment.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Educate patients and their families about health conditions, treatment options, and self-care techniques.
  • Assist with daily living activities and ensure a comfortable and supportive environment for patients.
  • Respond to patient concerns and changes in condition promptly and effectively.

Qualifications:
  • Valid LVN/LPN license in the state of CA
  • Current BLS/CPR certification.
  • Previous experience in a rehabilitation or long-term care setting preferred.
  • Strong clinical skills with the ability to work both independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Compassionate, patient-centered approach with a commitment to quality care.
