Job Summary

Oak Glen Post Acute is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our skilled nursing team. The LVN will provide high-quality nursing care to residents while working collaboratively with RNs, physicians, and interdisciplinary staff to support resident health, safety, and comfort.
Responsibilities

• Provide direct nursing care to residents in accordance with facility policies and procedures
• Administer medications and treatments as ordered by physicians
• Monitor residents' conditions and report changes to the RN or physician
• Accurately document care, treatments, and resident responses in medical records
• Assist with admissions, discharges, and care planning
• Ensure compliance with infection control, safety, and regulatory standards
• Support residents and families with professionalism, empathy, and respect
• Collaborate with the interdisciplinary care team to deliver person-centered care

Qualifications

• Current Licensed Vocational Nurse (LVN) license in the state of California
• CPR certification (BLS)
• Experience in skilled nursing or long-term care preferred
• Strong clinical, communication, and documentation skills
• Ability to work effectively in a fast-paced healthcare environment
• Commitment to providing compassionate, resident-centered care
