Position Responsibilities
  • Conduct comprehensive nutritional assessments for residents
  • Develop, implement, and monitor individualized nutrition care plans
  • Collaborate with physicians, nursing staff, speech therapy, and dietary services
  • Monitor weight trends, lab values, wounds, and overall nutritional status
  • Provide education to residents, families, and staff as needed
  • Ensure compliance with state, federal, and facility regulations
  • Participate in care plan meetings and interdisciplinary team discussions
  • Oversee therapeutic diets and ensure accurate diet implementation

Qualifications
  • Registered Dietitian (RD) credential, active and in good standing
  • Current state licensure (if applicable)
  • Experience in skilled nursing or long-term care preferred
  • Strong clinical assessment and documentation skills
  • Ability to work independently and as part of a team
  • Passion for improving the quality of life for the elderly
