Napa Valley Care Center is seeking a detail-oriented and organized MDS Assistant to support our clinical team in managing resident assessments and care documentation. If you have a strong understanding of skilled nursing documentation and a passion for accuracy and compliance, this could be the perfect fit!

Position Highlights:

• Assist the MDS Coordinator with timely and accurate completion of MDS assessments
• Ensure proper documentation to support coding and reimbursement
• Help monitor care plans and interdisciplinary team input
• Stay current on Medicare/Medicaid guidelines and PDPM standards

Qualifications:

• LVN license in CA
• Experience with MDS 3.0 in a skilled nursing setting strongly preferred
• Strong attention to detail and organization
• Familiarity with PDPM and CMS compliance requirements
• Team-focused mindset with excellent communication skills
