Your Responsibilities
- Evaluate and develop individualized treatment plans to improve resident function and mobility
- Collaborate with nursing, therapy, and medical teams to achieve optimal outcomes
- Document progress and adjust treatment as needed to support rehabilitation goals
- Provide education and support to residents and families
Qualifications
- Current California Physical Therapist (PT) license
- Skilled Nursing or post-acute experience preferred, but not required
- Compassionate, reliable, and committed to clinical excellence
