Pay: $38-$45/hour
Schedule: Full-Time

Napa Post Acute is looking for a knowledgeable and detail-oriented LVN MDS Coordinator to join our dedicated team. If you're passionate about quality care, thrive in a collaborative environment, and have a strong understanding of the MDS process, we'd love to connect with you.

What You'll Do:

• Coordinate and complete MDS assessments in accordance with federal and state regulations
• Ensure accurate and timely submissions
• Collaborate with interdisciplinary team members to support care planning
• Monitor documentation to ensure compliance and optimize reimbursement
• Support clinical staff with MDS-related processes and education

What We're Looking For:

• Current California LVN license
• MDS experience preferred (or strong clinical background with willingness to learn)
• Detail-oriented with strong organizational skills
• Ability to work independently while being a team player
• Knowledge of PDPM and regulatory requirements is a plus
