Social Services Director

Mission Valley Post Acute - Fremont, CA

Status: Full-Time

Salary: $85, 000 per year

Mission Valley Post Acute is seeking a dedicated, compassionate, and proactive Social Services Director to join our leadership team. In this role, you will be the primary advocate for our residents, ensuring their physical, mental, and psychosocial well-being is maintained at the highest level. You will act as the vital link between residents, families, and community resources to facilitate a seamless care experience.

Essential Duties & Responsibilities
  • Resident Advocacy: Assist residents in achieving the highest practicable level of self-care, independence, and emotional well-being.
  • Care Planning: Evaluate social and family information and psychological needs to develop comprehensive care plans; establish courses of action by exploring options and setting goals with families.
  • Emotional Support: Provide counseling and support for residents and families coping with physical disabilities, health fears, grief, and end-of-life transitions.
  • Resource Coordination: Secure community resources, health/welfare agency support, and appropriate prosthetics or assistive devices.
  • Ancillary Services: Schedule on-site services including optometry, podiatry, dentistry, and psychiatric care; assist with outpatient appointments.
  • Discharge Planning: Coordinate with the interdisciplinary team and outside agencies to ensure safe transitions, including arranging equipment and agency referrals.
  • Resident Services: Assist with personal shopping, inventory tracking of belongings, and investigating reports of missing items.
  • Collaboration: Maintain strong working relationships with all facility departments and provide staff in-service training as requested.


Qualifications
  • Education: Bachelor's Degree in Social Work or a related human services field (Required).
  • Interpersonal Skills: Exceptional listening and verbal communication skills; ability to build trust with residents and their families.
  • Professionalism: High degree of objectivity, confidentiality, and the ability to handle high-pressure situations with persistence.
  • Organization: Strong planning, reporting, and documentation skills; a proactive approach to problem-solving.
