Position Summary

Mission Valley Post Acute is seeking a compassionate and detail-oriented Case Manager to join our team on a part-time basis. The Case Manager plays a critical role in coordinating resident care, facilitating communication between interdisciplinary teams, and ensuring smooth transitions throughout the continuum of care.

Key Responsibilities
  • Coordinate and manage resident care plans in collaboration with nursing, therapy, and social services teams
  • Monitor resident progress and adjust care plans as needed
  • Facilitate discharge planning, including coordination with families, physicians, and outside agencies
  • Ensure compliance with all regulatory and documentation requirements
  • Maintain accurate and timely case management records
  • Act as a liaison between residents, families, and healthcare providers
  • Assist with insurance verification and authorization processes when applicable
  • Identify and address barriers to effective care and safe discharge
  • Participate in care conferences and interdisciplinary meetings


Qualifications
  • Previous experience in case management, healthcare coordination, or social services preferred
  • Knowledge of post-acute care processes and regulations is a plus
  • Strong organizational and multitasking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and collaboratively within a team
  • Proficiency in basic computer systems and documentation
