What You'll Do:

• Coordinate and oversee all social services functions in the facility
• Support residents and families through the admissions, care planning, and discharge process
• Conduct psychosocial assessments and develop individualized care plans
• Build strong relationships with residents, families, and community resources
• Help ensure compliance with state and federal regulations

What We're Looking For:

• Experience in a skilled nursing facility or healthcare setting preferred
• Strong organizational, communication, and problem-solving skills
• Knowledge of discharge planning, care transitions, and social services documentation
• Ability to multitask and maintain a calm, compassionate presence under pressure
