Midtown Oaks Post Acute is seeking a Full-Time Physical Therapist (PT) to join our collaborative rehab team in a skilled nursing/post-acute care setting. This is a great opportunity for a compassionate, motivated clinician who values quality patient care, teamwork, and work-life balance.

💰 Compensation
  • $60-$70/hour, based on experience

🩺 Physical Therapist (PT) Responsibilities
  • Evaluate and treat patients in a skilled nursing/post-acute environment
  • Develop and implement individualized treatment plans
  • Complete evaluations, progress notes, and discharge documentation accurately and on time
  • Collaborate with nursing, therapy, and interdisciplinary teams
  • Educate patients and families on therapy goals and home exercise programs

✅ Physical Therapist (PT) Requirements
  • Active California Physical Therapist license (or pending)
  • Skilled nursing or post-acute experience preferred, but not required
  • Strong communication, documentation, and time-management skills
  • Team-oriented, compassionate, and patient-centered approach
