In this role, you will help residents improve their communication, cognitive-linguistic, and swallowing abilities while collaborating with an interdisciplinary team dedicated to delivering exceptional patient care.
Key Responsibilities
Resident Evaluation & Treatment
- Assess residents to identify communication, cognitive, and swallowing deficits
- Develop and implement individualized treatment plans based on physician orders and clinical findings
- Monitor resident progress and modify treatment plans as appropriate
Interdisciplinary Collaboration
- Partner with physicians, nursing staff, families, and other healthcare professionals to coordinate care and optimize outcomes
- Participate in care planning meetings and discharge planning discussions
Education & Training
- Educate residents and caregivers on treatment goals, home programs, communication strategies, and swallowing safety
- Provide in-services and educational support to staff as needed
- Mentor Clinical Fellows (CFY), students, and rehabilitation team members when applicable
Documentation & Compliance
- Complete timely and accurate documentation in accordance with facility policies, regulatory standards, and payer requirements
- Maintain compliance with all federal, state, and facility guidelines
Quality & Safety
- Promote a safe treatment environment through adherence to infection control and safety procedures
- Support quality assurance and performance improvement initiatives
Qualifications
- Current State Speech-Language Pathologist (SLP) license
- Certificate of Clinical Competence (CCC-SLP) preferred
- Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred
- Strong communication, organizational, and teamwork skills
- Commitment to providing high-quality, resident-centered care
