Join Our Team as a Social Services Director!

McKinley Park Care Center is seeking a dynamic, compassionate, and organized Social Services Director to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about supporting residents and their families through every stage of care.

What You'll Do

• Develop individualized care plans by working closely with residents and their families
• Support residents in achieving the highest level of independence and well-being
• Provide medically related social services to enhance residents' physical, mental, and psychosocial health
• Assess emotional, psychological, and social needs of residents
• Offer counseling and emotional support to residents and families
• Assist with healthcare decision-making and coping with illness, disability, or loss
• Coordinate discharge planning with interdisciplinary teams and outside agencies
• Connect residents with community resources and support services
• Schedule outpatient appointments and coordinate ancillary services (optometry, podiatry, dental, psychiatric, etc.)
• Maintain accurate and timely documentation
• Manage resident belongings and respond to lost/missing item reports
• Provide staff in-service training as needed
• Build strong relationships with internal teams and external partners

What We're Looking For

• Bachelor's Degree in Social Work or Human Services required
• Minimum of 2 years of supervised social work experience in a healthcare setting
• MSW preferred
• Strong communication, organization, and problem-solving skills
• Ability to handle sensitive situations with professionalism and confidentiality
• A proactive, compassionate approach to resident care
