Key Responsibilities - Evaluate residents and create individualized treatment plans
- Provide therapy to improve strength, mobility, ADLs, and functional independence
- Collaborate with interdisciplinary team members on care plans and discharge goals
- Document treatments, progress, and outcomes accurately and timely
- Educate patients and families on home exercise programs and adaptive techniques
Qualifications - Current Occupational Therapist (OT) license in good standing
- Strong communication and teamwork skills
- Skilled nursing or post-acute experience preferred (not required)
