Key Responsibilities:

• Evaluate patients and develop individualized treatment plans to improve mobility and function
• Implement therapeutic exercises, techniques, and interventions based on patient needs
• Collaborate with a multidisciplinary team to ensure optimal patient care and progress
• Educate patients and caregivers on exercises and strategies for continued progress
• Maintain accurate and timely documentation of patient care and progress

Qualifications:

✅ Active Physical Therapist (PT) License
✅ Experience in a skilled nursing facility (SNF) or rehabilitation setting preferred
✅ Strong communication and interpersonal skills
✅ Passion for helping patients recover and improve their quality of life
✅ Ability to work independently and as part of a team
