Key Responsibilities- Complete and coordinate the Minimum Data Set (MDS) process in compliance with state, federal, and facility guidelines
- Accurately assess and document resident needs and conditions
- Collaborate with the interdisciplinary team to ensure care plans reflect resident goals and clinical requirements
- Monitor quality measures and ensure accurate reimbursement through proper coding and documentation
- Assist in training staff on MDS-related protocols and updates
- Support the Director of Nursing and leadership team in driving quality outcomes and regulatory compliance
Qualifications- Active California LVN or RN license required
- Minimum of 1-2 years of MDS experience in a skilled nursing facility preferred
- Strong knowledge of PDPM and care planning process
- Excellent clinical judgment, organizational skills, and attention to detail
- Strong communication and teamwork skills
- Passion for improving patient outcomes and ensuring accurate documentation
