Are you an experienced MDS Coordinator with a passion for excellence in resident care and a sharp eye for detail? Join Marin Post Acute, where your clinical expertise and leadership will directly shape outcomes—and where your hard work is truly appreciated.

We are a skilled nursing facility located in the heart of Marin County, and we are looking for a licensed LVN or RN with a proven track record in MDS coordination. If you thrive in a fast-paced, high-impact environment and are committed to quality care, we want to hear from you.

Why This Role Matters:

Working in post-acute and long-term care isn't just a job—it's a calling. You'll be helping vulnerable residents get the care they need while supporting their return to independence or providing comfort in their later years. The work is challenging—but incredibly rewarding. Every accurate assessment and well-crafted care plan makes a real difference in someone's life.

What We're Looking For:
  • Valid California LVN or RN license
  • Proven experience as an MDS Coordinator in a skilled nursing facility
  • Deep understanding of PDPM, care planning, and state/federal regulations
  • Ability to collaborate across interdisciplinary teams with clarity and confidence
  • Strong organizational and documentation skills

What We Offer:
  • Competitive pay: $50+/hr for LVNs and $60+/hr for RNs (based on experience)
  • Supportive leadership, including an amazing Administrator who genuinely cares about the staff and residents and fosters a positive, team-oriented culture
  • A stable, well-managed facility with an excellent reputation in the community
  • A collaborative clinical team that values your input and expertise
  • Opportunities for growth, mentorship, and continuing education

At Marin Post Acute, you'll never be just a number—you'll be a respected and valued member of our leadership team.
