What You'll Do
- Provide individualized physical therapy services to post-acute and long-term care residents
- Evaluate patients and develop customized treatment plans focused on functional outcomes
- Collaborate with an interdisciplinary team to support patient-centered goals
- Help patients improve strength, mobility, balance, and overall physical function
- Document treatment plans and progress in compliance with regulatory standards
- Educate patients and families on therapy goals, progress, and home exercise programs
- Current California Physical Therapist license in good standing
- Strong communication and teamwork skills
- Passion for working with the senior population
- Commitment to high-quality, patient-centered care
- Experience in a skilled nursing or post-acute setting preferred
