Position: MDS Coordinator (RN)
Location: Linwood Meadows
Part-Time, 2 days a week

Hourly rate $40-50 an hour BOE

Overview:
We are seeking an experienced and dedicated MDS Coordinator (RN) to join our team at a premier Skilled Nursing Facility in Visalia, Ca. The ideal candidate will be responsible for managing the Minimum Data Set (MDS) process, ensuring compliance with state and federal regulations while supporting optimal resident care and reimbursement practices.

Key Responsibilities:
  • Oversee and coordinate the timely and accurate completion of MDS assessments in accordance with federal and state guidelines.
  • Ensure proper documentation and accurate coding of resident care to support clinical and reimbursement goals.
  • Collaborate with interdisciplinary teams to develop, update, and implement comprehensive care plans for residents.
  • Provide education and guidance to staff regarding MDS processes, regulatory changes, and care planning.
  • Monitor and manage quality measures and identify areas for improvement within the facility.
  • Act as a liaison with regulatory agencies during surveys and audits related to MDS processes.

Qualifications:
  • Current and valid Registered Nurse (RN) license in the state of California.
  • Minimum of 2 years of experience in MDS coordination or a related role within a Skilled Nursing Facility.
  • Strong knowledge of Medicare and Medicaid reimbursement processes, RAI guidelines, and regulatory compliance.
  • Excellent organizational, communication, and leadership skills.
  • Proficiency in electronic health record (EHR) systems and MDS software.
