Pay Range: $28-$31 per hour
Position Summary

Linwood Meadows Care Center is seeking a dedicated Part-Time Licensed Vocational Nurse (LVN) to provide direct nursing care to residents and supervise the daily activities of Certified Nursing Assistants (CNAs).

The LVN ensures care is delivered in accordance with federal, state, and local regulations, as well as facility policies and direction from the Director of Nursing or Nurse Supervisor, to maintain the highest level of quality resident care.
Essential Duties and Responsibilities
Nursing Care & Clinical Responsibilities
  • Provide direct nursing care in accordance with physician orders and resident care plans.
  • Administer medications and treatments as prescribed.
  • Monitor residents' conditions and report changes to physicians and family members as appropriate.
  • Perform skilled nursing procedures including wound care, catheterization, tube feedings, suctioning, specimen collection, and other treatments as required.
  • Take and record vital signs and document resident status accurately and timely.
  • Participate in admissions, transfers, and discharges.
  • Assist with discharge planning and care coordination.
  • Ensure resident care is delivered in accordance with individualized care plans and facility standards.
Supervision & Leadership
  • Direct and supervise CNAs during assigned shifts.
  • Conduct shift rounds to ensure nursing care standards are maintained.
  • Provide guidance, support, and leadership to assigned nursing staff.
  • Communicate staffing needs or concerns to the Nurse Supervisor.
  • Promote a calm, respectful, and positive environment for residents and staff.
Documentation & Compliance
  • Accurately transcribe and implement physician orders.
  • Complete required documentation including nurses' notes, incident reports, and medication records.
  • Ensure all entries in resident medical records are timely, complete, signed, and dated.
  • Maintain confidentiality of resident information in compliance with HIPAA.
  • Participate in quality assurance activities and facility surveys.
  • Follow infection control, safety, and regulatory guidelines at all times.
Supervisory Responsibilities

As an LVN, you are delegated the authority, responsibility, and accountability necessary to supervise nursing assistants and ensure quality care during your assigned shift.
Qualifications
Education and Experience
  • Graduate of an approved LVN program.
  • Current, active, and unencumbered LVN license in the State of California.
  • Experience in a skilled nursing facility preferred.
Required Skills and Abilities
  • Strong clinical assessment and critical thinking skills.
  • Ability to make independent decisions when appropriate.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary teams.
  • Leadership and supervisory abilities.
  • Proficiency in electronic medical records; PCC experience preferred.
  • Ability to handle the physical and emotional demands of long-term care nursing.
Certificates & Licensure
  • Current LVN license in good standing.
  • Current CPR certification.
  • Must maintain required continuing education and licensure.
Physical Requirements
  • Ability to move throughout the facility during the workday.
  • Ability to lift, push, and pull at least 25 pounds.
  • Ability to assist with resident mobility and emergency situations if needed.
  • Ability to cope with the mental and emotional stress of a skilled nursing environment.
Work Environment

This position works throughout the nursing service areas including resident rooms, medication rooms, and nurses' stations. The role involves frequent interaction with residents, families, staff, and regulatory agencies. Exposure to infectious diseases and hazardous materials may occur.
