Lincoln Meadows Care Center is seeking compassionate Physical Therapists (PT) to join our dedicated team in providing high-quality care to our residents. We have part-time positions available with weekend availability required. Key Responsibilities:

• Provide physical or occupational therapy services to elderly residents
• Develop and implement personalized therapy plans to enhance resident mobility, independence, and quality of life
• Collaborate with a multidisciplinary team to ensure comprehensive care
• Maintain accurate documentation of therapy sessions and progress
• Support residents and their families through education and compassionate care

Qualifications:

• Valid Physical Therapist (PT) license in the state
• Experience in a skilled nursing facility or with elderly patients is a plus
• Strong interpersonal and communication skills
• Ability to work independently and as part of a collaborative team
