Lincoln Meadows Care Center

Lincoln Meadows Care Center is excited to announce an opening for a Full-Time Occupational Therapist to join our dedicated team. As an integral part of our rehabilitation team, you will work closely with residents to assess their needs and develop personalized therapy plans that enhance their independence and quality of life.

Key Responsibilities:
  • Conduct comprehensive assessments to determine the needs and goals of each resident.
  • Develop, implement, and evaluate individualized treatment plans to improve daily living skills, motor functions, and cognitive abilities.
  • Collaborate with a multidisciplinary team, including physicians, nurses, and other therapists, to ensure optimal care for each resident.
  • Document progress and adjust treatment plans as needed.
  • Educate residents and their families on therapeutic techniques and tools to improve quality of life.
  • Maintain compliance with all healthcare regulations and documentation standards.
  • Weekend availability required


Ideal Candidate:
  • Licensed Occupational Therapist with current state licensure.
  • Strong team player with excellent communication and interpersonal skills.
  • Passionate about providing compassionate, person-centered care.
  • Experience in a skilled nursing or long-term care facility is preferred, but not required.
  • Ability to work independently, with attention to detail and strong organizational skills.
