Hanford Post Acute, where we provide high-quality, person-centered care in a skilled nursing setting.
We are currently seeking a
Speech Language Pathologist (SLP) to evaluate, plan, and implement therapy services to meet the communication and swallowing needs of our residents.
Responsibilities Include: - Evaluate patients within 24 hours or 1 business day of physician referral (with documentation if delayed)
- Develop and implement personalized treatment plans approved by referring physician
- Provide direct treatment and document patient progress in compliance with state and facility standards
- Supervise SLP Assistants and Clinical Fellows as needed
- Participate in interdisciplinary meetings, care conferences, and in-service training
- Provide caregiver education and discharge planning
- Maintain accurate and timely documentation in systems like Casamba and PointClickCare
- Comply with all applicable laws, regulations, and facility policies
Qualifications: - Bachelor's degree in Speech-Language Pathology required; Master's or Doctorate preferred
- Active and unencumbered California SLP license
- CPR certification and continuing education per state requirements
- Strong clinical reasoning and interpersonal communication skills
- Experience in SNF setting preferred
- Proficient in electronic documentation systems (e.g., Casamba, PCC)
Physical Demands & Work Environment: - Frequent standing, walking, reaching, and lifting up to 50 lbs
- Occasional stooping, kneeling, or crouching
- Moderate noise level typical of a healthcare facility
