MDS Nurse (LVN) - Full-Time

Salary: $70, 600 annually (Salaried)
Status: Full-Time
General Purpose

The primary purpose of the MDS Nurse (LVN) position is to oversee the facility's resident assessment process to ensure all assessments are completed accurately, timely, and in compliance with federal, state, and payer guidelines. This role focuses on individual resident assessments, MDS completion, PDPM processes, and coordination of interdisciplinary care planning to support optimal resident outcomes.
Essential Duties & Responsibilities
  • Provide oversight of the facility's weekly PDPM meetings for skilled patients
  • Track and monitor effective utilization of PDPM tools, including but not limited to:
    • Physician Diagnosis Verification
    • Admission/Discharge CSR (GG)
    • Weekly CSR
    • PDPM Weekly Tracking Tool
    • PointRight
  • Track, monitor, and analyze QM and QASP performance monthly and quarterly; provide clinical recommendations for improved outcomes
  • Provide PDPM and MDS training to new MDS Coordinators
  • Assess and determine the health status and appropriate level of care for all new admissions
  • Ensure accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments
  • Communicate resident level of care to all interdisciplinary team members
  • Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each admission in accordance with regulatory timeframes
  • Maintain an accurate schedule of all MDS assessments, including proper reference dates throughout the resident's stay
  • Ensure completeness, accuracy, and regulatory compliance of all MDS documentation
  • Perform data entry and electronic submission of MDS assessments; verify submissions and complete corrections as needed
  • Schedule and conduct resident care conferences in compliance with state and federal regulations
  • Assist disciplines in developing, revising, and evaluating resident care plans to ensure individualized, goal-oriented care
  • Evaluate achievement of desired resident outcomes and ensure care plans are revised appropriately
  • Manage and communicate all level of care changes to applicable departments
  • Generate and transmit required forms related to acuity and level-of-care changes in accordance with state law
  • Maintain confidentiality of all resident information, including Protected Health Information (PHI)
Supervisory Requirements

This position has supervisory responsibilities.
Qualifications

Education and/or Experience
  • Graduation from an Accredited School of Nursing
  • LVN license required (Bachelor of Science in Nursing preferred)
  • Minimum 3 years of supervisory experience preferred
  • Long-Term Care experience strongly preferred
  • Must be in good standing with the State Board of Nursing and maintain all required continuing education and licensure
Certificates, Licenses & Registrations
  • Active California LVN license in good standing
  • Proficiency in Microsoft Office Suite
  • Knowledge and experience with PCC preferred
  • Must maintain all required certifications and licensure
