Position Summary

As a Physical Therapist, you will evaluate, plan, and implement individualized treatment programs for residents in a skilled nursing and post-acute setting. You'll play a key role in helping patients recover from injury, surgery, or illness while maximizing their mobility and independence.

Key Responsibilities
  • Conduct comprehensive patient evaluations and develop customized care plans
  • Provide therapeutic exercises, manual therapy, and mobility training
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with physicians, nursing staff, and therapy team members
  • Educate patients and families on treatment plans and home exercises
  • Maintain accurate and timely documentation in compliance with regulations


Qualifications
  • Active Physical Therapist (PT) license in California
  • Graduate of an accredited Physical Therapy program
  • Strong clinical assessment and communication skills
  • Experience in skilled nursing or post-acute care preferred, but not required
  • Commitment to compassionate, high-quality patient care
