Position Details

• Location: Concord, CA (Contra Costa County)
• Pay: Starting at $60/hour DOE
• Schedule Options: Full-Time, Part-Time, and PRN available
• Flexible Scheduling: Choose from Tuesday-Saturday or Sunday-Thursday schedules
• New Graduates Welcome: Structured 1-on-1 mentorship provided

Key Responsibilities

• Conduct evaluations and create individualized treatment plans in collaboration with physicians
• Supervise Physical Therapist Assistants in accordance with California State Practice Act guidelines
• Collaborate with nursing and interdisciplinary teams to support patient progress and discharge planning
• Recommend appropriate DME and support safe patient transitions home
• Educate patients, families, and staff on safety, mobility, and therapeutic programs

Qualifications

• Current California Physical Therapist (PT) license required
