Diablo Valley Post Acute in Concord is looking for a passionate and motivated Physical Therapist to join our dynamic in-house rehabilitation team. If you're seeking a collaborative environment that values clinical excellence, flexibility, and professional growth, we'd love to meet you.
As a trusted provider of short-term rehabilitation and long-term skilled nursing care, we offer a fast-paced, patient-centered therapy program operating seven days a week. Our team is committed to delivering exceptional outcomes while fostering a positive, supportive workplace culture.
Position Details- Location: Concord, CA (Contra Costa County)
- Pay: Starting at $60/hour DOE
- Schedule Options: Full-Time, Part-Time, and PRN available
- Flexible Scheduling: Choose from Tuesday-Saturday or Sunday-Thursday schedules

Key Responsibilities- Conduct evaluations and create individualized treatment plans in collaboration with physicians
- Supervise Physical Therapist Assistants in accordance with California State Practice Act guidelines
- Collaborate with nursing and interdisciplinary teams to support patient progress and discharge planning
- Recommend appropriate DME and support safe patient transitions home
- Educate patients, families, and staff on safety, mobility, and therapeutic programs

Qualifications- Current California Physical Therapist (PT) license required
