Physical Therapist (PT) to join our premier rehabilitation team. As a leading provider of short-term recovery and long-term care, we operate a 7-day-a-week therapy program focused on high-quality outcomes and interdisciplinary success.
The Opportunity: - Position: Physical Therapist (PT)
- Location: Concord, CA (Contra Costa County)
- Compensation: Starting at $60.00 per hour (DOE)
- Availability: Full-Time, Part-Time, and PRN
- Shifts: Flexible 5-day schedules (Tue-Sat or Sun-Thu) — Skip the Monday rush!

Your Impact & Responsibilities: - Clinical Leadership: Perform comprehensive evaluations and develop physician-approved treatment plans tailored to each resident's recovery journey.
- Team Supervision: Provide direct patient care and expert supervision of Physical Therapy Assistants (PTAs) in accordance with California State Practice Acts.
- Interdisciplinary Teamwork: Collaborate with the medical and nursing teams to communicate patient progress, goals, and discharge planning.
- Resident Advocacy: Lead discharge planning and recommend necessary durable medical equipment (DME) to ensure a safe transition home.
- Education: Provide vital education to nursing staff, patients, and families regarding long-term maintenance and caregiver safety.
- Documentation Excellence: Maintain compliant, precise, and timely documentation of all clinical services.
