Physical Therapist (PT) - Diablo Valley Post Acute

Location: Concord, CA

Salary: Starting at $60hr

About the Facility: Diablo Valley Post Acute is a leading provider of short-term rehabilitation and long-term care. We operate a 7-day-a-week therapy program, ensuring our patients receive consistent, high-quality care. We are seeking a licensed PT who is dedicated to clinical excellence and interdisciplinary collaboration.

Clinical Responsibilities:

  • Perform comprehensive evaluations and develop physician-approved treatment plans.
  • Direct patient care and supervision of Physical Therapy Assistants (PTAs) in accordance with CA state practice acts.
  • Collaborate with the interdisciplinary team to communicate patient progress and goals.
  • Provide education to nursing staff, patients, and families regarding maintenance and caregiving.
  • Lead discharge planning and recommend necessary durable medical equipment (DME).
  • Maintain compliant and timely documentation of all clinical services.
