Position Details
- Position: Physical Therapist (PT) - PRN
- Location: Concord, CA
- Compensation: Starting at $64-$65/hour
- Schedule: PRN / Per-Diem
- Environment: Supportive, team-oriented, and fast-paced rehab setting
Key Responsibilities
- Perform comprehensive physical therapy evaluations in a timely manner following physician referral
- Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management, and functional independence
- Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure quality patient care and compliance with state practice standards
- Participate in interdisciplinary care planning, patient conferences, and utilization review meetings
- Recommend appropriate Durable Medical Equipment (DME) to support safe mobility and discharge planning
- Educate residents, families, and caregivers on exercise programs, safety strategies, and long-term maintenance plans
- Maintain accurate and timely documentation in accordance with regulatory and payer guidelines
Qualifications
- Current California Physical Therapist (PT) license in good standing
