Your Impact & Responsibilities:

  • Rapid Evaluation: Conduct comprehensive patient evaluations within 24 hours (or 1 business day) of physician referral to jumpstart the recovery process.
  • Evidence-Based Treatment: Develop and implement individualized plans focused on strength, balance, pain management, and functional mobility.
  • Clinical Leadership: Supervise Physical Therapy Assistants (PTAs) in direct patient care, ensuring strict adherence to the California State Practice Act.
  • Interdisciplinary Synergy: Participate in Patient Care Conferences and Utilization Review (UR) meetings to ensure seamless transitions and integrated care.
  • Equipment Advocacy: Recommend and facilitate the ordering of Durable Medical Equipment (DME) to enhance resident safety and independence.
  • Empowerment: Educate patients, families, and nursing staff on maintenance programs and caregiver training in preparation for a successful discharge.
  • Precision Documentation: Maintain accurate, timely evaluations and progress notes in accordance with state and payer requirements.


Qualifications:

  • Licensure: Current and in good standing California Physical Therapist (PT) license.
  • Skills: Strong communication, interpersonal, and time-management skills for effective caseload management.
  • Mindset: Compassion, empathy, and a "patient-first" approach to rehabilitation.
