Citrus Heights Post Acute is looking for a dedicated and compassionate Speech-Language Pathologist (SLP) to join our team! You'll play a vital role in helping patients regain communication, swallowing, and cognitive function in a collaborative, supportive skilled nursing environment.

What You'll Do

• Evaluate patients within 24 hours (or 1 business day) of physician referral
• Develop and implement individualized treatment plans
• Provide therapy per physician orders
• Monitor, document, and adjust patient progress as needed
• Collaborate with physicians, nursing staff, and interdisciplinary team members
• Participate in care conferences, rehab meetings, and discharge planning
• Educate patients, families, and caregivers for continued care
• Provide in-services and training for facility staff
• Maintain accurate documentation using EMR systems

Qualifications

• Master's or Doctorate degree in Speech-Language Pathology
• Active California SLP license (required)
• Strong clinical, communication, and documentation skills
• Skilled nursing or post-acute experience preferred, but not required
• Team-oriented with the ability to thrive in a fast-paced environment

Certifications & Requirements

• Current SLP license in good standing with the state
• Ongoing continuing education per state requirements
• Familiarity with EMR systems
