Position Overview:
We are currently seeking dedicated and experienced Registered Nurses to join our team. As a Registered Nurse at Citrus Heights Post Acute, you will play a crucial role in delivering high-quality care to our residents. You will collaborate with a multidisciplinary team to assess, plan, implement, and evaluate individualized care plans. Responsibilities:

• Conduct comprehensive assessments of residents' health status
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments as prescribed
• Monitor and document residents' progress and responses to treatment
• Provide education and support to residents and their families
• Maintain accurate and up-to-date medical records
• Adhere to all facility policies, procedures, and regulatory requirements

Qualifications:

• Valid Registered Nurse (RN) license in the state of California
• Minimum of 1-2 years of experience in a long-term care or post-acute setting preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work effectively as part of a multidisciplinary team
• Commitment to providing compassionate and person-centered care
• Basic computer skills and proficiency in electronic medical records systems
