Full Job Description
Canyon Springs Post-Acute is a 199-bed skilled nursing and short-term rehabilitation facility. We are seeking a full-time RN Supervisor to oversee daily clinical floor operations, manage staff, and ensure excellent resident care.
Responsibilities:
- Supervise daily floor nursing staff and assign clinical duties.
- Lead morning stand-up meetings to coordinate team communication.
- Update residents and families on Care Plans, PT, OT, and treatment paths.
- Answer clinical, care, and illness progression questions to reassure residents.
- Assess and adjust patient care statuses based on changing clinical needs.
- Conduct utilization reviews to provide timely, cost-effective care.
- Complete accurate, timely charting and paperwork using PointClickCare (PCC).
- Manage admissions, discharge planning, and external facility referrals.
- Coordinate benefits with financial services, Medicare, Medi-Cal, and third-party payers.
Requirements:
- Active California Registered Nurse (RN) license.
- Minimum 2 years of clinical nursing experience (supervisory experience preferred).
- Knowledge of Medicare, Medi-Cal, and Medicaid programs.
- Proficiency with PointClickCare (PCC) or electronic medical records software.
- Excellent communication, problem-solving, and time management skills.
- Good standing with the California Department of Public Health.
- Ability to stand/walk frequently and lift up to 50 lbs.
