Your Clinical Impact:

  • Evaluation & Planning: Conduct thorough resident evaluations and design evidence-based treatment plans tailored to individual recovery goals.
  • Direct Treatment: Deliver high-quality, hands-on therapy in accordance with the California State Practice Act.
  • Clinical Leadership: Supervise Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care delivery.
  • Precise Documentation: Maintain accurate daily notes, progress reports, and recertification documentation in a timely manner.
  • Interdisciplinary Collaboration: Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.
  • Discharge Success: Educate residents and families on safe mobility and home-exercise programs; recommend and coordinate necessary DME (walkers, braces, etc.).


Qualifications:

  • Licensure: Valid, unrestricted California Physical Therapist (PT) License.
  • Skills: Strong clinical reasoning, accurate documentation, and the ability to motivate residents through their recovery journey.
  • Relationship Building: A positive attitude and the ability to build rapport with residents, families, and professional colleagues.
  • Passion: A genuine commitment to helping others regain their strength and independence.
