Canyon Springs Post Acute is seeking a motivated

The Opportunity: - Position: PRN (As Needed)
- Rate: Starting at $65.00+ per hour
- Location: Canyon Springs Post Acute - San Jose, CA

Your Clinical Impact: - Evaluation & Planning: Conduct thorough resident evaluations and design evidence-based treatment plans tailored to individual recovery goals.
- Direct Treatment: Deliver high-quality, hands-on therapy in accordance with the California State Practice Act.
- Clinical Leadership: Supervise Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care delivery.
- Precise Documentation: Maintain accurate daily notes, progress reports, and recertification documentation in a timely manner.
- Interdisciplinary Collaboration: Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.
- Discharge Success: Educate residents and families on safe mobility and home-exercise programs; recommend and coordinate necessary DME (walkers, braces, etc.).
Qualifications: - Licensure: Valid, unrestricted California Physical Therapist (PT) License.
- Skills: Strong clinical reasoning, accurate documentation, and the ability to motivate residents through their recovery journey.
- Relationship Building: A positive attitude and the ability to build rapport with residents, families, and professional colleagues.
