Your Impact & Responsibilities:

  • Functional Evaluations: Assess residents' functional status and develop goal-oriented treatment plans to improve Activities of Daily Living (ADLs).
  • Skilled Intervention: Deliver evidence-based occupational therapy to enhance independence, safety, and quality of life.
  • Interdisciplinary Collaboration: Work seamlessly with PTs, SLPs, and nursing staff to ensure a holistic approach to resident wellness.
  • Accurate Documentation: Maintain timely and compliant electronic documentation to track resident progress.
  • Discharge Planning: Empower residents and families with adaptive strategies and home program education for a safe transition home.


Qualifications:

  • License: Current, valid California Occupational Therapist (OTR) license.
  • Expertise: Strong clinical reasoning and a passion for geriatric or subacute rehabilitation.
  • Reliability: A proactive professional who thrives in a fast-paced, resident-centered setting.
