Position Summary
- Coordinate and complete MDS assessments to ensure compliance with federal and state regulations
- Maintain accurate clinical documentation to support reimbursement and care planning
- Collaborate with interdisciplinary teams to improve resident outcomes
Key Responsibilities
- Complete MDS/CAA assessments on time, ensure compliance, and submit for reimbursement
- Collaborate on care plans, monitor resident progress, and participate in care conferences
- Support audits, train staff on documentation, and stay current on regulatory updates
Qualifications
- Active California LVN/LPN license and experience in skilled nursing or long-term care preferred
- Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines
- Strong organizational, teamwork, and EHR/Microsoft Office skills

- Effective communication skills with residents, families, and care teams
- Ability to interpret policies, procedures, and clinical documentation requirements
- Basic math, problem-solving, and attention to detail
- License Required: Active LVN/LPN (California)
