Aviara Healthcare Center Is looking for a compassionate, committed, and skilled
Physical Therapist to join our team!
Purpose: Act as the patient's partner to restore and maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
Duties and Responsibilities: - Identify and meet patients' needs and goals
- Develop care plans using a variety of treatment techniques
- Create fitness- and wellness-oriented programs tailored to patients' specific needs
- Provide quality, personalized and evidence-based care and proven interventions
- Motivate patients during treatment in order to help them function optimally
- Reduce the need for medications and provide alternatives to surgery
- Evaluate effects and monitor and communicate progress
- Document patient care services
- Supervises Physical Therapy assistants according to organization policy and state regulations.
Requirements and Qualifications: - Bachelor's or Master's degree in Physical therapy approved by an accredited organization
- State Licensed to practice as a Physical Therapist
- Experience as a Physical Therapist in a SNF is a plus
- Possesses and maintains current CPR certification
Physical Capacities: (With or Without the Aid of Mechanical Devices)
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
- Must be able to move intermittently throughout the workday.
- Continuous bending, squatting, twisting and reaching.
- Must meet the general health requirements set forth by the policies of this facility which includes an annual TB screening and physical examination.
- Must be able to stand for and walk for extended periods of time.
- Must be able to lift and carry up to 25 pounds.
- Requires infrequent lifting of weight up to 100+ pounds with assistance.
- Frequent pushing and pulling of carts and transporting residents.
- Must be able to cope with the mental and emotional stress of the position.
Environmental conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, and potential exposure to bloodborne pathogens.
