Job Posting: Part-Time Case Manager (Non-Licensed)
Location: Arvin Post Acute
Schedule: Part-Time (24-30 hours/week)
Starting Pay: $16.58/hour

We are seeking a highly organized and communicative Case Manager to join our team on a part-time basis. This non-licensed position plays a vital role in supporting residents and their families by coordinating care services and facilitating communication across departments.
Key Responsibilities:
  • Communicate regularly with residents and families regarding Plans of Care, therapy schedules, and treatment updates
  • Attend and participate in daily stand-up meetings to facilitate team communication
  • Answer resident questions about care plans, services, and facility routines to ensure understanding and comfort
  • Monitor resident needs and status changes; update appropriate team members
  • Coordinate admissions, discharges, and referrals to/from the facility
  • Work closely with therapy, nursing, and financial departments to ensure continuity of care
  • Assist with utilization reviews and ensure care is timely, appropriate, and cost-effective
  • Accurately complete required documentation in a timely manner
  • Maintain professional, respectful communication with residents, families, and staff
Qualifications:
  • High school diploma or equivalent (required)
  • Associate's or Bachelor's degree in Social Services, Healthcare Administration, or related field preferred
  • Experience in a healthcare or long-term care setting strongly preferred
  • Knowledge of Medicare and Medi-Cal/Medicaid programs is a plus
  • Strong organizational, time management, and communication skills
  • Ability to work independently and manage multiple priorities in a fast-paced environment
  • Intermediate computer skills required; knowledge of PCC preferred
Physical Requirements:
  • Frequent standing, walking, and lifting up to 50 lbs
  • Occasional bending, kneeling, and reaching
  • Ability to maintain focus and attention to detail in a moderately active environment
