Description:

Part-Time - Any 3 weekdays

Pay rate: $40-53/hr
  • Provides oversight on weekly PDPM meetings and ensures facilities effectively utilize tools such as Physician Diagnosis Verification, CSR assessments, and Point Right.
  • Tracks and analyzes QM and QASP performance monthly and quarterly, offering clinical recommendations to improve outcomes.
  • Ensures accurate and timely completion of MDS assessments, including Medicare PPS, quarterly, annual, and significant change.
  • Coordinates interdisciplinary teams to complete MDS assessments within regulatory time frames and ensures thorough documentation.
  • Maintains and manages accurate MDS schedules, data entry, and electronic submission; performs corrections as needed.
  • Trains and mentors new MDS Coordinators on PDPM and MDS processes to ensure compliance and competency.
  • Schedules and conducts resident care conferences in accordance with regulations, ensuring MDS reviews are complete beforehand.
  • Assesses health status and level of care for all new admissions, communicates care levels, and updates departments as needed.
  • Oversees level-of-care changes, generating appropriate documentation and ensuring submission to state agencies.
  • Maintains confidentiality of all resident care and health information in compliance with HIPAA and related regulations.
