Description:
Part-Time - Any 3 weekdaysPay rate: $40-53/hr- Provides oversight on weekly PDPM meetings and ensures facilities effectively utilize tools such as Physician Diagnosis Verification, CSR assessments, and Point Right.
- Tracks and analyzes QM and QASP performance monthly and quarterly, offering clinical recommendations to improve outcomes.
- Ensures accurate and timely completion of MDS assessments, including Medicare PPS, quarterly, annual, and significant change.
- Coordinates interdisciplinary teams to complete MDS assessments within regulatory time frames and ensures thorough documentation.
- Maintains and manages accurate MDS schedules, data entry, and electronic submission; performs corrections as needed.
- Trains and mentors new MDS Coordinators on PDPM and MDS processes to ensure compliance and competency.
- Schedules and conducts resident care conferences in accordance with regulations, ensuring MDS reviews are complete beforehand.
- Assesses health status and level of care for all new admissions, communicates care levels, and updates departments as needed.
- Oversees level-of-care changes, generating appropriate documentation and ensuring submission to state agencies.
- Maintains confidentiality of all resident care and health information in compliance with HIPAA and related regulations.
