The Opportunity:
  • Position: On Call (LVN)
  • Location: All Saints Subacute & Transitional Care
  • Rate: $35.00 per hour

Your Impact & Responsibilities:
  • Bedside Excellence: Provide high-quality nursing care, including medication administration, wound treatments, and resident assessments.
  • Clinical Monitoring: Closely monitor resident vitals and conditions, promptly reporting any changes to the RN or physician.
  • Care Coordination: Collaborate with our in-house therapy team (PT/OT/SLP) and physicians to ensure comprehensive recovery plans.
  • Documentation: Maintain precise, timely, and compliant electronic medical records (PointClickCare experience is highly valued).
  • Leadership: Provide direction and mentorship to CNAs to ensure resident ADLs are met with the highest standards of care.
  • Advocacy: Act as a compassionate resource for residents and their families during their transitional care journey.


Qualifications:
  • License: Must possess a current, valid California Licensed Vocational Nurse (LVN) license.
  • Certification: Current CPR/BLS Certification.
  • Experience: Previous experience in subacute, skilled nursing, or transitional care is preferred.
  • Skillset: Strong clinical judgment, excellent communication skills, and a "team-first" attitude.
