Responsibilities:

• Conduct comprehensive physical therapy evaluations to assess residents' mobility, strength, and overall functional abilities.
• Develop and implement personalized treatment plans to help residents achieve their rehabilitation goals.
• Provide hands-on physical therapy treatments, including therapeutic exercises, manual therapy, and use of specialized equipment.
• Collaborate with an interdisciplinary team, including physicians, nurses, and other therapists, to ensure coordinated and effective care.
• Educate residents and their families on home exercise programs and strategies to maintain progress after discharge.
• Document treatment sessions, progress notes, and discharge plans accurately and timely.

Qualifications:

• Current California Physical Therapist license.
• Strong clinical skills with a focus on patient-centered care.
• Excellent communication and interpersonal abilities.
• Ability to work effectively both independently and as part of a team.
• Experience in a skilled nursing or rehabilitation setting is preferred, but not required.
