Westminster Village, Inc. is hiring a Charge Nurse & Care Coordinator HCC in Scottsdale, AZ. This is a full-time position.
POSITION SUMMARY: The Charge Nurse provides leadership and oversight for daily clinical operations within the assisted living community, ensuring resident care aligns with service plans, regulatory standards, and Westminster Villages commitment to dignity and independence. Working under the direction of the Assisted Living Nurse Manager and Assisted Living Manager, the Charge Nurse supervises caregivers and medication technicians, fosters effective communication among residents, families, and healthcare providers, and responds promptly to changes in resident condition. This role combines hands-on nursing care with coordination, problem-solving, and staff development to support a safe, engaging, and person-centered environment. ESSENTIAL RESPONSIBLITY: Performs and monitors direct care that aligns with resident service plans. Assists with the supervision of resident care personnel, including caregivers and medication technicians. May assist, when necessary, with medication administration and wound care, that aligns with physician orders. Ensures there is effective communication with residents, family members, and medical practitioners, and addresses immediate concerns, as necessary. Recognizes changes in resident conditions and implements appropriate critical thinking and nursing care. Participates and helps to lead regular staff meetings and staff development, as required.Consults with Nursing Manager, Assisted Living Manager, and other disciplines to provide optimal care. Uses time effectively on the floor to promote meaningful engagement in care and activities. Assists with the coordination of facility admissions, discharges, and transfers. Supports in addressing concerns that arise on the floor before escalating to a supervisor and educates on the continuum of care and facility resources Effectively promotes purpose through engagement of visitor, staff member, and residents in unit and facility activities. Displays a positive, resident-focused demeanor that emphasizes independence, autonomy, and dignity. Performs other duties or responsibilities as assigned by the Nursing Manager or the Assisted Living Manger. EDUCATION/EXPERIENCE: High school graduate or equivalent.Valid, good standing LPN license through the Arizona Board of Nursing.Current fingerprint clearance card through the Arizona Department of Public Safety.Current CPR and First Aid certification.Memory care training, as required by the Department of Health Services (Or able to receive within 30 days of hire).Previous experience in an assisted living setting is preferred. PHYSICAL DEMAND: Able to sit, stand, bend, lift and move intermittently during working hours. Can lift up to 40 pounds. Can sustain prolonged standing and walking. *All qualified applicants must successfully pass a background check, APS registry check, substance test, fingerprinting requirements, memory care training, and tuberculosis testing. #High
