Key Responsibilities

Clinical Care & Medication
  • Administer prescribed medications and treatments safely.
  • Monitor residents' health conditions and document changes.
  • Complete accurate and timely documentation in EMR systems (PCC preferred).
  • Transcribe physician orders and update care plans accordingly.
  • Supervise and guide CNAs and nursing assistants on your shift.
  • Conduct rounds to ensure quality care and adherence to procedures.

Qualifications
  • Valid LPN/LVN license in Arizona (unencumbered).
  • Current CPR certification.
  • Graduate of an accredited LPN program.
  • Long-term care or assisted living experience preferred.
  • Proficiency in EMR systems (PointClickCare a plus).


Work Environment
  • Fast-paced, hands-on caregiving environment.
  • Requires standing, walking, and lifting up to 25 lbs.
  • Subject to exposure to infectious materials, chemicals, and emotionally demanding situations.
  • May require working evenings, weekends, holidays, and during emergencies.
