Job Summary Manages the day-to-day clinical services of a more complex community to ensure residents' activities of
daily living (ADLs) and healthcare needs are met. Verifies residents are treated with respect and dignity
and verifies residents are receiving quality care as their needs change. Supervises and provides leadership,
as well as coaching, to licensed nurses and other direct care staff within the community, while creating an
inclusive community culture that provides high quality resident experiences and care, and engages
residents, families, and associates.
May be responsible for leading additional clinical leadership team up to five members. The HWD level
for each community is determined based on the total complexity of the role. Complexity criteria include,
but are not limited to, factors such as size, type of product lines, medication management regulations, 90-
day assessment requirements, multiple licensure requirements, state regulatory complexity, and skilled
services requiring an RN.
Essential Functions - Directly supervises community-based licensed, certified, and non-certified clinical
associates. - Partners and collaborates with district team leaders to verify that day-to-day clinical
services meet Company standards and applicable state regulations. - Leads care associates and community to gain a reputation for best in market ADL
care. - Acts as a hands-on leader who supervises, directs, and motivates clinical associates.
Attracts, develops, engages, and retains associates in accordance with Company
policies. Recruits and hires high quality and engaged direct care staff while
confirming they are appropriately trained and developed to meet the needs of
residents. - Reviews promotions, development plans, disciplinary actions, and termination
decisions ensuring consistency in the selection and retention of quality direct care
staff. Encourages teamwork and collaboration, and cultivates an inclusive community
culture, while maximizing labor productivity and efficiencies. Analyzes trends and
implements strategies to reduce turnover and increase retention of direct care staff,
appropriately minimizing premium labor. - Provides training and education to direct care staff on an ongoing basis with
classroom in-services and situation-specific training. - Supervises the maintenance of resident charts and reviews documentation performed
by clinical staff. - As described and allowed in the Nurse Practice Act and as required by state
regulation, proactively manages care and services for each resident, evaluates
effectiveness, and maximizes the resident's opportunity to remain in their
environment. Participates in pre-admission screening of prospective new residents.
Confirms that required documentation is completed prior to or upon resident
admission. - Manages communication to residents and residents' responsible party regarding care
needs and healthcare appointments. - Assesses potential new residents and educates referral sources on the community's
care/acuity guidelines for admission. Engages with healthcare providers in care
coordination for residents to increase understanding and visibility of Brookdale's
services in the local market. - Performs on-site evaluations of residents admitted to alternate care environments for
treatment, and maintains contact with resident families with the intent of returning
resident to Brookdale, as appropriate. Facilitates continuity of care for those residents
receiving home health care, hospice services, and other third party healthcare-related
services. - Evaluates residents, documents changes in condition, and notifies Executive Director,
resident's physician, and resident's legally responsible party/family of resident's
condition and reactions. Confirms in-house ancillary medical services (i.e., doctor
visits, dental visits, psych visits, lab, X-ray, ambulance, etc.) are scheduled and
completed as necessary. - Participates in or leads meetings relevant to resident care issues, such as Service Plan
meetings, involving appropriate parties as needed. - Assists Executive Director with resident retention through participation in new
resident welcome events, resident referral programs, Creating a Resident Experience
(CARE) program, and collaborative care review process. - Participates in department quality improvement activities.
- Routinely evaluates resident transfer and mobility needs, and verifies clinical staff
are aware of any changes. - Shares on-call for clinical care concerns and manager on duty (MOD) responsibilities
with other key department leaders as required. - Enforces current Company policies and procedures. Maintains applicable licenses in
accordance with Company, Federal, State, and local requirements.

Education and Experience - Education as required to obtain state nursing license (LPN/LVN or RN).
- Minimum of 3 years clinical leadership experience preferred.
- Working knowledge of basic technology and electronic documentation.
- Skilled nursing experience may be required.
Certifications, Licenses, and other Special Requirements State nursing license (LPN/LVN or RN). Must have valid driver's license.

the Company's policies and applicable laws.
Knowledge and Skills Possesses extensive knowledge of necessary clinical skills. Effective written and oral communications
skills are essential. Familiarity with Microsoft software preferred. Ability to prioritize and effectively
manage multiple tasks is essential. Ability to delegate assignments to the appropriate individuals based on
their skills, roles, and interests. Demonstrates good judgment and strong problem solving and decision
making skills.
