Are you an organized, compassionate Registered Nurse who enjoys collaborating with others and helping ensure residents receive the highest quality of care? NHC HealthCare Anniston is seeking an MDS Coordinator RN to join our team.

In this role, you will work closely with residents, families, and our interdisciplinary care team to complete accurate assessments and develop timely, individualized care plans.
Key Responsibilities
  • Conduct and document accurate resident assessments
  • Interview residents and family members to gather important care information
  • Collaborate with the interdisciplinary team to ensure resident assessment protocols and care plans are completed on time
  • Maintain the MDS calendar and help ensure all departments meet required completion deadlines
  • Participate in on-call responsibilities as needed
Qualifications
  • Current Registered Nurse (RN) license required
  • Strong communication, organization, and time-management skills
  • Caring, compassionate, and dependable
  • Positive attitude and team-oriented approach
  • Previous RN experience preferred
  • MDS and PDPM experience preferred
  • RAC or CMAC certification preferred
