We are seeking a passionate, creative, and resident-focused Activities Director to lead a dynamic activity program in a skilled nursing facility. The Activities Director plans, develops, organizes, implements, evaluates, and directs a comprehensive, resident-centered activity program while ensuring compliance with all applicable federal, state, and professional standards.

Key Responsibilities
  • Plan, develop, organize, implement, evaluate, and direct resident-centered activity programs in compliance with CMS regulations and state requirements.
  • Create and maintain a monthly activity calendar that includes therapeutic group activities, resident outings, and individualized in-room programming for bed-bound or isolated residents.
  • Collaborate with the interdisciplinary team to develop and implement resident assessments and individualized activity care plans.
  • Design activity interventions that reflect resident interests, abilities, functional levels, and psychosocial needs.
  • Encourage resident independence and participation in self-directed activities such as hobbies, crafts, reading, and social engagement.
  • Provide adaptive materials as needed, including large print, Braille resources, and audio materials.
  • Ensure activity documentation and progress notes are timely, accurate, descriptive, and reflective of resident response and outcomes.
  • Maintain effective communication with residents, families, staff, volunteers, community partners, and regulatory agencies.
  • Stay current on federal and state regulations, survey guidance, and professional standards related to activity services in skilled nursing facilities.
  • Participate in survey readiness efforts and assist with plans of correction related to activity-related deficiencies; provide written reports to the Administrator as needed.
  • Support community planning, outreach initiatives, and facility events that enhance resident engagement and community connection.


Qualifications
  • High school diploma or equivalent required; college coursework in recreation, social services, or a related field preferred.
  • Qualified Activities Professional per federal and state requirements (NCCAP certification or eligibility preferred/required per state).
  • Prior experience in a skilled nursing or long-term care setting preferred.
  • Working knowledge of CMS regulations, including F-Tags related to Activities and Quality of Life.
  • Strong organizational, communication, and documentation skills.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • A creative, compassionate, and resident-centered approach to care.
